Participating Sites

A core strategy of the CHART Project has been to leverage exisiting experience and resources of Boston Medical Center to transfer EMR technology to community settings at an affordable cost. The sharing of clinical templates (the elements in the EMR that structure data collection during clinical visits) is another critical component. The pediatric templates developed over 6 years at BMC are being used in the CHART Project. Over the coming years, the CHART practices, in collaboration with BMC, will continue to enhance this common set of clinical templates. The project seeks to create a propective EMR-based primary care database with data of "high granularity" and capable of evaluating a broad range of primary care process and outcome measures.

CHART is comprised of BMC and 8 community health centers (CHC). All of the CHART CHCs are Federally Qualified Community Health Centers (FQHC) and/or Bureau of Primary Health Care (BPHC) 330/340 designated centers. All the sites are located in Governor Designated Health Professional Shortage Areas (GDHPSA) and serve predominantly poor populations of minority ethnicities. All provide comprehencsive primary care services to children and adults.

BMC and CHART Site Descriptions:

Boston Medical Center (BMC): BMC is a private, not for profit, 547-licensed bed academic medical center. The hospital is the primary teaching affiliate for Boston University School of Medicine and the largest safety net hospital in New England.  BMC has 17 attending pediatric faculty physicians, 10 family medicine attending faculty physicians, 25 pediatric resident physicians, 6 family medicine resident physicians, and 4 nurse practitioners.  The clinics have over 35,000 patient visits annually for children and adolescents. 

Codman Square Health Center (CSHC): The CSHC is located in Dorchester, MA, has over 62,000 visits annually, 3 pediatric clinicians, 2 family practice clinicians, and provides ambulatory training of residents. 

Dorchester House Multicultural Center (DHMSC): The DHMSC is located in Dorchester, a neighborhood of Boston, has over 77,000 ambulatory patient visits per year, 9 pediatric clinicians (including nurse practitioners) and provides ambulatory training of pediatric residents. 

Greater Roslindale Medical and Dental Center(GRMDC): The GRMDC primary serves the Roslindale, Hyde Park, and West Roxbury communities of Boston.  The GRMDC has over 17,000 visits annually, has 1 pediatric clinician, 2 family practice clinicians, and offers training for pediatric residents. 

Harvard Street Neighborhood Health Center (HSNHC): HSNHC is located in Roxbury, MA and primarily serves the North Dorchester, Mattapan and Roxbury communities of Boston. The HSNHC has over 85,000 visits annually, four pediatricians, and offers training of pediatric residents. 

Health Care for the Homeless (HCH):  HCH provides comprehensive services to homeless families and children in the Boston area.  One pediatrician and 1 nurse practitioner are available for the care of children.

Mattapan Community Health Center (MCHC): The MCHC is located in Mattapan, MA and has over 24,000 visits annually, 3 pediatricians, and offers training of pediatric residents. 

South Boston Community Health Center (SBCHC): The SBCHC is located in South Boston and provides comprehensive primary health care services for families and individuals living in the surrounding area.  The SBCHS has over 70,000 patient visits annually, has 4 child health clinicians (pediatricians and family medicine), and offers training to pediatric residents.

Whittier Street Health Center (WSHC): The WSHC is located in Roxbury and primary serves the Roxbury and Dorchester communities of Boston.  The WSHC has over 40,000 patient visits annually, has 3 pediatric clinicians, and provides training for pediatric residents.
